LZW36 Missing Medium

I have a standard ceiling fan with pull chains set to high.

When controlling the fan through z-wave or from the switch, it doesn’t seem like there’s a medium setting. 66 via SmartThings seems to be “low” (maybe slightly slower than low) and 67 seems to be “high.”

Any idea what the issue could be?

On Hubitat, 33% is slow, 66% is medium, and 99% is high. 1% is auto (breeze mode)…
