I replaced everything with a new LZW36 out of the box and everything seems to be working fine with this switch. I guess the other one was either defective or I caused an issue with it.
Question is what do I do with the non working switch?
I have another two of these to do with identical wiring and have one ariving Saturday hopefully this one will be good.
